Residents of Stockton-on-Tees can be referred by their GP’s, Practice Nurses, Physiotherapists or other Health Professionals to either the Lite 4 Life: Weight Management Referral Programme or the Active Health: Exercise Referral Programme. Both programmes aim to improve aspects of your lifestyle; the person who refers you will be able to give advice on which is suitable for you.
Lite 4 Life is designed specifically for people who are overweight or obese and want to lose weight through a change in their diet and physical activity levels. If your body mass index is 25+ and a medical professional deems that your are suitable to be referred you can access the following course:
The programme is a group session and is one hour a week that runs over ten weeks. The session consists of individual weigh-ins and weight management advice, covering different topics each week.
What's Included...Active Health is designed for people who have a medical condition (not directly related to being overweight or obese) which can be improved by increasing their physical activity levels.
